Intermittant problem

Every once in a while, when I boot up, explorer.exe fails to start. I can Ctrl/Alt/Del to open "Task Manager" and start it from there and everything is fine. I've thoroughly scanned for spyware/viruses. This might not happen for 2 weeks and then happen every day for 3-4 days and stop again for an extended period. Can't figure it out, no new updates in a month, no new software, no new printers. Anyone ever see this?

While in Safe Mode try these.

Anything could be causing Explorer to crash. You might consider recent updates. Also consider any other changes you have made. Corrupted files and malware can cause this too.

See if System Restore will get you back to a restore point before your problem with Explorer.

Try running ChkDsk to check your drive for errors. Right click your Drive icon/ Properties/ Tools/ Error Checking.

I have run chkdsk, nothing, I also ran a third party disk checker also clean.I have scanned for all sorts of malware/spyware/adware/viruses using a pretty extensive list of programs.
The problem with system restore is I don't know when it began, and since it disappears for weeks at a time it could have been a while ago.
I suspect an update too, but without a date to go by I am stuck unless I want to delete the latest one and wait a few weeks then rinse and repeat.
I was hoping that someone else had experienced this immediately after making some change to their system.
btw explorer doesn't crash, it just never starts, no errors in any log files and process explorer sees nothing unusual.

I've seen similar behavior but not quite that intermitant. Once gave an MSGINA error in the log and the other just kinda sat there staring at me.

In both cases I was able to correct the problem by reinstalling a SP. One time I used the XP SP2 and the otehr was actually an IE6 SP.

Give those a try and see if it starts behaving.

There may be a failed call that explorer depends on.
It is weird that it would be intermitant. You may want to try a shell repair with this;

Event log is identical

I compared the event logs (and process explorer) between days when it hung and days when it did not and they are absolutely identical. It almost acts like a timing thing. If I boot up in any other mode I've never seen it happen, not that that means anything.
I have been putting off SP3 for a while, guess I'll just have to bite the bullet and install it.

I have Symantec Ghost Configuration Server installed on my machine. Every few months I use it to ghost critical machines on the network. After I do that the Ghost server looks to reconnect to all of the clients every time I boot up. If even one of the clients is off line it just sits there waiting. I turned off the Ghost server and all appears to be well. Until I need to use ghost again.
