Depends on how hard you want to look. When I was first given the task of comin gup with an Internet and Email policy, I went to public library and signed up to used their computers. I made a copy of the statements I had to sign there, revised themfor my needs, and the deed was done.

These days talk to a lawyer. It highly depends on the type of company you work for.
Use the demo they have here and submit it to your corp. attourney. They will appricaite the chance to review something they will probably have to defend, and all said and done your boss will feel more secure when he sees how much of the language the attourney will change to suit your business.

ELECTRONIC MEDIA COMMUNICATION AND INTERNET POLICY
All electronic communication systems and all communications and information transmitted by, received from, or stored in these systems are the property of the company and as such are to be used solely for job-related purposes. The use of any software or business equipment, including, but not limited to, facsimiles, telecopiers, computers and copy machines, for private purposes (including games, and other entertainment features) is strictly prohibited. Transmitting or displaying messages or pictures of a pornographic, sexist, racist or otherwise offensive nature is also prohibited. Employees using this equipment for personal purposes do so at their own risk. Further, employees are not permitted to use a code, access a file or retrieve any stored communication unless authorized by an appropriate management representative. No employee may use a pass code that has not been issued to that employee or that is unknown to the company. Employees who violate this policy are subject to disciplinary action, up to and including discharge.To ensure that the use of electronic communications systems and business equipment is consistent with our legitimate business interests, authorized representatives of the company may monitor the use of such equipment from time to time.
Ownership owns and retains the right to examine any information on any equipment owned or used by PeopleWorks. Employees of PeopleWorks should not expect e-mail or other electronic storage to be private. PeopleWorks may, from time to time, examine e-mail messages or other contents of an employee?s computer. Propriety: Electronic media should be treated as any other media. Anything which would be inappropriate in any other media is also inappropriate in electronic media. Games and Recreational Activities: Games and other recreational activities are not allowed during business hours.
