Is this MCSE test question wrong or am I missing something?

Yes, I got the answer wrong; I was just trying to see what I needed to brush up on and I couldn’t remember if the threshold at which Windows Defrag recommends a defrag was 10% fragmentation or 15%. But shouldn’t drive G, at 41% fragmentation, trigger a recommendation to defrag too? Or am I missing something?

  Not by the percentage, the percentage can get skewed if say you have several virtual machines hard drives, a 128gb file in two pieces will not cause a noticeable performance hit, but can cause a very high percentage fragmentation. And it is unlikely that defrag will be able to defrag this file anyways.
