Monitoring Consent Forms

I would like to know if anyone can give me an idea of what (legally) needs to be included in a form that I need to develop for the recording (monitoring) of incoming calls.

This consent form is going to be signed by my help desk specialists before they are hired and I want to cover as many bases as possible.

Well, the FCC has some laws in regards to recording phone conversations. Basically all that has to be done to be "legal" though is make aware through any type of written documentation in advance to your users that they can be recorded at any time and you also need to make sure that the people calling in are aware as well. Most companies handle this by having a recording on their wait line that simply notifies all callers that for "customer service reasons" their conversation could be recorded or monitored. If recording the conversation and verbal notice is given it needs to actually be on the recording at the beginning. All states and circumstances are different. Ask your lawyer. Then pay him/her big $$$$$.
