Need a pop up window or application

I am in need of a application window or html window that when your computer is booted this window opens up in the top right corner of the screen. This window will contain 3 or 4 fields.
One field will be a text input and the other 3 just check boxes. When you hit the submit button in this small window it will update a table in a database. This window should not be able to close and or if it is closed it will pop back up again into the same position. Does anyone know how or what application I can design this in?

There are several ways of doing this.

One that comes to mind involves creating a quick web page that does what you need regarding the input and then updating of a database.

Once you have that created and working you simply set the desktops to use that web site as your background.

You can then lock out users from changing this setting so it will always be availible from their desktop.

The idea of creating the web site is a good one. Instead of adding it to the Desktop youcould put it in the Start up folder. You would need to use JavaScript to disable the close or minimize button until the form is filled out. You would also need to make sure that JavaScript it turned on for your website, and that the user could not change any of these settings. This can be done in System Policies.

You could also right a visual basic or C# or C++ or delphi application that runs on start up to interact with the database, I recommend using VB if it is for an MS database. And you can customise the window so that it will always remain.
