Need help merging Excel appointment calendar to patient letters (Word).

I have an Excel worksheet which lists a year worth of appointments (Rows listing dates and Column A listing times). I need a merge code/formula that will identify the appointment times for each last name listed and merge the appointments into a list for a letter in Word. Last year I used this same set up, but manually added each patient's appointments to the trailign columns on our patient list spreadsheet. This felt redundant and took time. I'm self taught in Excel and Word, and the obstacles I see with this request are the lack of frequency the date rows appear in the spreadsheet (so I'm not sure how to reference the date cell since it's not just Row 1). Not sure if this makes it impossible, hence me asking you experts.

HERE is link to Test Excel file (with sample last names). The actual file will list all Mondays and Tuesday for all of 2015. "X" means not available: TEST CALENDAR.xls?dl=0

HERE is link to the sample letter. I have a patient list (.xls) I can merge the patient's name & address from, and then that 'LAST NAME' field result would have to be used to query the 2015 appt list.xls to generate the list of appointments in the letter. APPOINTMENTS MERGE LETTER TEST.doc?dl=0

If anyone has any other solution, please share. I did preview excel-4-business's scheduler but I don't need 80% of that system. Thanks in advance for your time.
