Need MS Access Command Button help

I am in a serious bind to get a MS Access Form Completed in Access 2000. The Form Currently has the following data objects: 2 drop down field selectors, 1 ActiveX Date/Calender Field using the pre-defined Calendar for forms in MS Access. 3 Basic Fields all together (2 of them drop downs, 1 a regular text box with a date that is supplied from the Calendar.)

I am trying to create a COMMAND button that will take call current Fields as they are entered or selected, and append them to a Sub-Form (Not sure if that is what is called, a embedded database that is in the footer of the Form). Using the basic COMMAND button wizard is getting me no where near to entering records into a NEW or EXISITING table within the database.

I am looking for specific instructions for appending/adding records into a new TABLE or an existing table with a command button on a form. I need specific instructions or a web page reference please. (I get confused easily.)
