Outlook over HTTP clients slow on outside after ISA 2004 install

We have a W2003 SBS box running Exchange 2003 with three clients on the outside with laptops and Outlook over HTTP. The server was behind a hardware firewall, and the clients worked fine. After ISA installation, Outlook connects but is extremely slow.

The server's ip info:
internal ip 192.168.0.1
dns server 192.168.0.1 (yes DNS server is running and configured)
external ip 10.235.253.2

We have a certificate from Equifax that lists the server name as "mail.domain.com" (replace "domain" with the real domain name). Therefore, I had to create an A record in DNS on the server to point mail.domain.com to 192.168.0.1. Also, the ISA web publishing rules are set to direct to mail.domain.com rather than publishing.domain.local. OWA, RWW, etc work fine.

When I look at the ISA logging when connecting to Outlook over HTTP from the outside, I see a few initial connections on port 443 to 192.168.0.1 that alternately succeed, then fail. Then I see a bunch of connections to 10.235.253.2:443. I suspect this is the problem, but I don't know why it's "failing" over to the external NIC. Help please!
