PBX or PC phone question

Here's what I'm trying to do (using free software, but will pay if I need to)....

I want to be able to call my computer from my cell phone. Then use my computer to use my home phone line to make a phone call.

The end result I want is to use my cell phone from anywhere and have it appear as if i'm calling from my land line.

Is this possible by using PBX software, or does anyone know of a program that will do this.

Some PBX Systems Do This Well...

We use an 'enterprise class' Avaya PBX system and they call this feature "extension to cellular" (EC-500). In their system your cell phone and your desk phone extension work seamlessly (your computer is not involved). When EC-500 is turned on any call coming into your land line rings at the same time on your cell. Any call from your cell appears to come from your land line...
Other PBX and even "unified communications" vendors including Microsoft offer this feature under many names and minor flavor changes.
