PC Migration

How can I migrate pc machines, user accounts, and mailboxes from an NT 4.0 domain and Exchange 5.5 environment to a Windows 2003 AD and Exchange 2007 (or Exchange 2003) environment? Please let me know. I haven’t migrated in a while and some of this stuff I’ve forgotten.
