physical memo dump

i am facing this prob alot these days, whenever for installing xp the setup files are copied to hard and rebooted i get msgs like ” physical memory dump, plz restart or some hardware is not installed properly ” ihave even formatted the partitons, but didnt help ,
plus what to do if i forget the password for xp and cant remember by the hint question , do ihave to reinstallit always
