Prince2 Healthcheck and lessons learned

guys,

i want to use a prince2 healthcheck to check my project and to create a lessons elarned log. I found some templates but i still dont know how to use those. How can i create a lessons learned form a healthcheck where you only have to add a yes or no? Some1 an idea or is can some one tell me if a real case is available somewhere for download?
