Questionaire

I need to compose a short questionaire for our help desk. The boss wants something that will help determine user, software or hardware fault. I'm stumped... Can anyone help with some ideas?

We are stumped also because you failed to mention which Application(s) OR Operating System(s) software you are talking about AND you didn't mention wether your talking about a LAN, WAN, Domain - or what???

Do you trap errors????

if you are using windows your best bet would
be to use microsoft works there is a built
in questionaire thing that will help you set
one up there is many templates that are very
useful that come with works if you need
questions then you willneed to be more
specific with your information but when you
get your questions all lined up go with works
because it will set everything up for you
