If your requirement is just to allow clients to use Exchange for email, Outlook Web Access works nicely: It lets client access their Exchange mailbox from e.g. Internet Explorer. To have them change their NT passwords, you also need to install SSL on the Exchange Server's IIS.

For more general access, consider Microsoft Dial Up Networking (RAS), to support VPN access, which will allow clients to access your Intranet via their ISP connections.

If the remote clients are together (e.g. in a remote office) you could link the sites together using a site-to-site VPN accross the Internet, using Microsoft or other vendors' VPN technology.

When connecting internal networks to the Internet, you will be advised to install a properly configured firewall to secure each site.
