if you use iis and have smtp properly installed on your sever,simply use "CDONTS.NewMail" a sample code like below:
Set objNewMail = CreateObject("CDONTS.NewMail")
strSVEmail="destination email address"
objNewMail.From = "source email address"
objNewMail.To = strSVEmail
objNewMail.BodyFormat=0
objNewMail.MailFormat=0
objNewMail.Subject = " "& strSubject 'your subject
objNewMail.Body = strBody 'your mail body generated by the form you submitted
objNewMail.Send
Set objNewMail=Nothing

If you don't have an NT service provider or if they restrict the type of server side scripts running then other alternative is too see whether your ISP offers a formmail script. You will need to find out what name it is and what parameters it takes,but this should take all your form info and send it to you as an email.
Usually your form will need to start with this:
<FORM METHOD="POST" ACTION="http://www.yourdomain.com/cgi-bin/
formmail.cgi">

and the chances are you will need to add somedetails about the email account you will receive this at, as well as adding some specific hidden fields.

You can use history object like follows:

<a href="javascript:history.go(-X)"></a>,
where X is number in history array. 1 is last link, 2 is second and so on.

<a href="javascript:history.back">,<a href="javascript:history.forward"> - links forhistory browsing.
