Setting Values in Outlook Form

I am building an Outllook 2010 form based on the message template. The form uses checkboxes to indicate maintenance items that need to be done. The data deson’t need to be saved or compiled anywhere – it just needs to be seen by the recipient. When I send the form, the recipient can’t see the check boxes or labels.

I suspect my problem is that I’m not binding these controls to a value. My question is, which field do I need to choose, or do I need to create my own field? Again, I only need the recipient to be able to see which boxes are checked and the labels above the boxes. I don’t need the data stored.
