SQL Server Change Management

I am being put in charge of overseeing the SQL Server at work. There has never been any change control on the server. I am trying to learn what other people do for change control. I downloaded the change control form, but I feel that is not enough. I would like to know what kind of change control policies other people are using. Who creates the databases? Once it is created, do you let the developers create their own tables? These are the things that I am looking for. Any help would be greatly appreciated.
