Strange Error Message on NW 4.1 Server

I have a customer with the following configuration:

Netware 4.1 Server
Adaptec AHA-2940au SCSI card [SCSI 2]
ArcservIt 6.6 backup software
HP T4000e tape backup drive - Firmware 1.07

I continue to get the following error message randomly about three times a week.

Board ASPI Board 00 id 04 SCSI 10 Host Adapter Error=04

When I have this situation I can down the server, but cannot exit back to DOS 6.22. Therefore, I must physically shut off power and then reboot. Reboot is accomplished without error. The other problem that happens concurrently with the error message is that the tape drive will not recognize a correct tape in my rotation set, therebye stopping daily backup of server. I can't get anyone to define error message.
For HP Tape Drive only: I have downloaded 1.10 firmware update for T4000, but when I try to update it stops with the following message:

SCSI library initialize failed

What does this mean?
Any Info would be appreciated.

Strange Error Message on NW 4.1 Server

Yesterday we had a similary problem in a similar configuration. We just turned P&P of the SCSI off and replaced the T4000 with an DDS-2 DAT4mm drive and everything worked fine.
Have always the latest TAPELIST loaded (downloadable from: ENTERPRISE:http://support.cai.com/Download/patches/asitnwee.html
WORKGROUP: http://support.cai.com/Download/patches/asitnwwg.html)

Exactly look what the last tape is asking for. Just after backup he telling which tape he will use next time. Use one of thattapes. Always write down the serial number of a tape on the tape label.
Also the next tape to be used can be found by Job Queue in ARCserve Manager. One of the tabs on the left right hand gives information about the next time. My advise is not to use rotion jobs, but just full backups.

Actually this is a question to get some more information.. What service Pack for Novell are you running?

Also in Startup.ncf what does you load statment for the driver look like?

LOAD AHA2940.HAM SLOT=10001 LUN_ENABLE=FF
