Submit form doesn't jump to right URL

I'm a newbie and I'm having one of the most common problems of all. When I submit my form, I don't arrive at my desried URL. Instead I get a message saying that the URL I'm trying to reach doesn't exist.

My host is Tripod and I'm working with a module called TripodPage.pm. Here's the code for my form, I'm sure the problem is fairly obivious. Thanks in advance for any help you can provide.

URLs don't appear to be correct

I doesn't look like the URL in the action attribute of your form tag is correct - if you copy and paste it into a browser, you get the "not found" message. Same with the URL in the value of the end_display field.

Since they don't work by typing them in, they're not going to work if submitted by a form. Figure out what the correct URLs should be and you're on your way.
