Top-level domains in my web stats — how does this work?

I’m sure this is a really really newbie question, please bear with.

When I look at my web stats, I see IP addresses as well as top-level
domains. I undertand the IP addresses; but how does a person surf the
web from their own domain?

And how much does it cost? Because that’s pretty sweet.
