Why Does Microsoft Exchange Use Only Public IP’s?

We had a server set set up running Windows Server 2000. During the configuration, my Tech’s said that Windows Exchange could only work through a public IP.

The ISP hosting our domain would not give us a public IP, so we eventually installed a 3rd party mail server.

Is it through that Microsoft Exchange can only run on public IP’s? If so, why is Microsoft sticking to this in the face of competition from 3rd party software that avoids the security risk of public IP’s?
