Worst Outsource Project Management

I am wondering, what is the worst outsource project management stories you have heard?

Here's mine:
I am currently involved in an outsourced project for a customer that is a major organisation that looks like it's going to break apart. The short story is, the sales people basically handed in the tender proposal without having once actually understanding the customer's environment. From what I hear, they did not even step on the customer's site once. They have committed to the customer's SLA, and proposed a model of operation which looks like a carbon copy of another's project's.

It's basically a case of coming up with the tool without seeing what the job is about.

Typically, in an outsourcing situation, the client is looking for low cost and to be relieved of the complexity of IT responsibilities. Outsourcing companies are usually eager to get the business and skip over the analysis. Both sides get sloppy and these deals get cut. Sometimes, if the numbers are way off, the deal gets renegotiated.

Sometimes I think the clients really don't care that much because as long as they get control of IT costs (which they have usually tried, and failed to control to their satisfaction) they can live with whatever pain the deal generates. Obviously, there will be individual business customers that are infuriated, but the bottom line starts to look better. It's sort of like a forced diet. Lower cost and lower service levels.
