" I.T." Work Request form

Has anyone out there created a form that users can "grab" with Outlook, fill in the blanks and send to "IT" to request work. I'm a 1.5 person shop and I need to get a more formal way of having work requests submitted.
suggestions?

pretty sure you can get this free from microsoft. also check out the free access sample work order database. search for 'sample' at support.microsoft.com

I've been searching Microsoft and did see the Access DB but don't want to create another monster in our office. Just want a nice form to get users to enumerate their needs.

I have created over 15 forms using MS Outlook, MS Access and MS Word (print forms). We average about 300 forms filled out each day using these templates.

A first nobody wanted t use the forms, they wanted to continue using the paper form. Now, if we talk about replacing the forms with something better we are threaten with a revolution.
