Best Practices for responding to Faults

I'm on a team who's mission is to set policy on when, who, why, etc. is to respond to network alerts of equipment faults from our monitoring system. Does anyone know of any "best practices" documents/books/articles on this subject?

humm usually policy is decided by committee with HR getting their big hands on the pie. Who is responsible usually lands on either the low man on the totem pole [which whoever is responsible to make sure nothing is messed up, ought to have faith in who will respond]. On the other hand, the FNG usually gets the job and if no one particularly likes him, he always gets the job cuz they hope he'll mess up and won't be the FNG but will be the FFG .
