I am in need of direction! I have been ask to provide an electronic forms solution for our customers, i.e. a downloadable form that can be fill in, saved and emailed back to the originator. For example: a doctors office needs to send out a form to patients to be filled out and emailed back prior to an appointment. Not absolutely necessary to be saved on the patients end and is not (at this time) tied into a database at the doctors end, just needs to be printed. I would really appreciate any help you might be able to provide.
