Check the front panel to see if LED remains on,this indicate a bad floppy
Check the cable on the inside of the computer connected to floppy,it may need to be replaced.
Click Start->Settings->Control Panel, then double-click the System icon, click the plus sign next to floppy drive and see if their is a yellow exclamation mark next to it,which will indicate a problem with the floppy
You may need to reinstall the floppy drive controller driver, which can be downloaded from the internet.

It could also be that the cable is plugged in backward on the floppy drive or the motherboard.

Does this happen only on write actions, or on both read & write?

If write only, make sure that the slide on the disk is not set to write protect.

Also, if the write protect switch is set in the bios, you cannot write to the floppy.

Otherwise, check device manager to make sure that their are no problems with the floppy drive.

Try removing the drive, and letting Windows re-install it.
Make sure that under the performance tab on device manager that it is set to detect new floppy drives (if available).
Also, when you re-boot, make sure the switch in the BIOS settings is set to report new floppy drives to Windows.

Windows should then find the drive, and re-install it.

It is also possible that the drive has gone bad, or is extreemly dirty.
